Osteoporosis literally means porous bones. It is a term used to describe a group of bone disorders in which the absolute bone volume (bone mass) is less than normal. This condition leads to fragile bones. In Osteoporosis, the bone mass is decreased, indicating that the rate of bone resorption exceeds that of bone formation. A person with osteoporosis has a higher risk of bone fractures particularly in the hip, wrist and spine.

Osteoporosis Symptoms

- Other significant factors leading to the onset of osteoporosis include hypothyroidism, anticonvulsant medications, a sedentary lifestyle, smoking, inadequate physical activity (weight bearing exercise), and family history.
Diagnosis of Osteoporosis
- Osteoporosis is most commonly discovered either on routine examination, or following a pathologic fracture (a fracture due to weakened bone).
- Densiometry or DEXA scan is the most useful screening test to detect osteoporosis. While this test does require special equipment, it is safe. It exposes the patient to small amounts of radiation, and is useful in detecting early stages of osteoporosis.
- X-rays usually show a generalized loss of bone density.
- Laboratory studies are not very helpful in diagnosing osteoporosis, but they are helpful in showing evidence of conditions that can have symptoms similar to osteoporosis. Laboratory studies can show evidence of osteomalacia, kidney failure, parathyroid gland insufficiency, or other problems that may mimic osteoporosis and cause bone weakness.
Osteoporosis Treatment
The primary goal of treatment of osteoporosis is to reduce the risk of pathologic fractures.
The three mainstays of treatment are:
1) Weight-bearing exercises:
Weight-bearing exercises and movements are prescribed for osteoporosis. Exercise cannot replace bone mass that has already been lost, but it can strengthen the bones. Simple movements can bring flexibility and agility to the joints. Agility helps to maintain balance and prevent falls.
While resistance exercise (e.g. with light weights) are excellent to stimulate bone production, even light aerobic exercises (e.g. aerobics, jogging, walking) can help strengthen bones and prevent osteoporosis.
2) Nutritional supplementation:
Calcium supplementation is an important treatment. The daily recommended intake should be at least 1200 to 1500 mg per day. Obtaining this amount of calcium through natural sources is difficult, considering that it is equivalent to drinking 5 glasses of milk each day. Due to this, patients at risk of developing osteoporosis are advised to take an oral calcium supplement every day. This should be taken with Vitamin D (400-800 Units is recommended) to help increase its absorption. 3) Medications:
Several different medications are used for treating osteoporosis. These include:
Bisphosphanates are a type of medication that help regulate calcium retention and prevent bone resorption.
Calcitonin is a naturally occurring hormone produced by the thyroid gland. It can be given in an injection or in the form of a nasal spray. It inhibits the function of osteoclasts - the cells that break down bone. Calcitonin has long since been known to benefit patients with osteoporosis, but the injections were difficult to administer and had unpleasant side-effects. The nasal spray has greatly improved the use of calcitonin, and it is much more commonly used today.
Raloxifene is a newer medication that has been developed to provide some of the same advantages of estrogen (HRT), without the potential side-effects. Raloxifene is a type of medication called a Selective Estrogen Receptor Modulator, or SERM. Estrogen Hormonal replacement therapy, or HRT, helps maintain, and even increase bone mass, after menopause. Multiple studies have shown the benefits of estrogen therapy as including a lowered risk of osteoporosis and fractured bones. HRT was shown to increase the risk of uterine cancer, but this risk is eliminated when the estrogen is combined with progesterone. Certain studies in some study populations show an increased risk of breast cancer with HRT. Patients on HRT have also shown a slightly increased risk of developing blood-clots and strokes.
Prevention of Osteoporosis
As discussed, there are several methods that can help slow down the otherwise rapid progression of osteoporosis. It is of utmost importance to remain active, even in the later years of life.
