Causes

The esophageal opening in the diaphragm is surrounded by muscles which prevent regurgitation of stomach content into the esophagus. Weakness of these muscles leads to herniation of the stomach through the hiatus. Such weakness is congenital, or may be acquired as a result of general laxity of muscle tone due to sedentary habits and increasing age.

Raised intra-abdominal pressure secondary to pregnancy, obesity, fluid retention or abdominal tumor is frequently responsible for Hiatus Hernia.

Types of Hiatus Hernia

The following varieties of Eesophageal Hiatal Hernias have been recognized:

1) Sliding Variety: This is the type most commonly seen. The esophago-gastric junction is drawn into the thorax above the opening of the diaphragm and can slide back into the abdomen. There is free gastro-esophageal reflux, which causes symptoms of inflammation of the esophagus.

2) Rolling Variety: The esophago-gastric junction is not drawn up in such cases. Only the stomach herniates into the thorax. This kind of hernia does not produce any symptoms.

3) Mixed type: Here, the features of both sliding and rolling varieties are seen together.

4) Hiatus Hernia with a short esophagus: This is very rare. Some cases are due to a congenital short esophagus, while others are secondary to inflammation of the esophagus, where it contracts and is pulled up into the thorax.

Signs and Symptoms

At times, Hiatus Hernia may be entirely asymptomatic.

Symptoms, when they occur, may be grouped into the following headings:

1) Symptoms due to gastro-esophageal reflux: Inflammation of the esophagus is caused due to regurgitation of the acid contents of the stomach into the esophagus.

Mild epigastric pain occurs soon after meals on lying down or on stooping forward and is relieved by sitting up or standing. Pain may radiate to the back, left shoulder and arm.

It may lead to vomiting of blood or passing of blood in stools. Sometimes, a stricture of the lower end of the esophagus is formed.

2) Anemia: A slow seepage of blood from the ulcerated inflamed lower-end of the esophagus, or from the congested mucosa of the herniated portion of the stomach may produce tiredness and weakness due t anemia.

3) Pressure symptoms: A large rolling kind of hernia can produce breathlessness, increased heart rate and palpitations, on lying down. All these symptoms are relieved on sitting up.

4) Symptoms due to strangulation or obstruction of herniated contents. These are rarely seen.

Diagnosis

A history of upper abdominal discomfort occurring on lying down or stooping forward and relieved by sitting up or standing is characteristic of a Hiatus Hernia.

Diagnosis can be confirmed by the following procedures:

-- Barium X-ray: In this procedure, the patient has to drink liquid containing barium. This provides an outline of the esophagus , stomach and intestines on an X-ray. It helps establish if the contents of the stomach regurgitate into the esophagus.

-- Endoscopy: In this procedure, a thin tube with a camera system fitted at the tip is passed down the throat into the esophagus and stomach. It gives a clear picture about the position of the stomach and the esophageal lining.
