Causes

Diminished renal excretion of uric acid

In more than 75% of patients with gout, there appears to be a genetically determined defect in urate excretion, which results in an inability to increase uric acid excretion.

Factors predisposing one to diminished excretion are renal failure, drugs, lead poisoning, myxedema, Down's syndrome, hyperparathyroidism and lactic acidosis.

Increased production of uric acid

This is the cause of gout in 25% of cases. In the absence of significant renal impairment, such patients are hyperexcretors of uric acid.

Factors predisposing one to increased production of uric acid are increased turnover of purines, myeloproliferative diseases like polycythaemia, lymphoproliferative diseases like leukemia, psoriasis and enzyme deficiencies.

Signs and Symptoms

The metatarso-phalangeal joint of the big toe is the site of the first attack. The ankles, knees, small joints of the feet and hands, wrist and elbow, generally follow in that order.

The onset may be insidious or explosively sudden, often waking the patient up from sleep.

The affected joint is red, hot and swollen with shiny overlying skin and dilated veins. It is excruciatingly painful and tender. Very acute attacks may be associated with fever, leucocytosis and a raised ESR.

Resolution of the acute attack may be accompanied by itching and desquamation.

Some patients have only a single attack, or suffer from another attack after an interval of many months or years. More often, there is a tendency to have recurrent attacks.

Acute attacks may be provoked by trauma, unusual physical exercise, surgery or severe systemic illness.

Diagnosis

Gout should be suspected in any case of acute monoarticular arthritis. This is more so in the case of the involvement of the big toe. Uric acid levels above 7mg/dl in men and 6mg/dl in women are highly suggestive of gout.

Serum urate level is usually raised, but it is important to appreciate that this does not prove the diagnosis, because asymptomatic hyperuricaemia is very common.

Whenever possible, synovial fluid should be aspirated and examined under polarizing light for uric acid crystals.

Joint radiographs are seldom useful in establishing the diagnosis.

Treatment

Conventional Therapy

NSAIDs are the agents of choice. It is important to start treatment as early as possible, to use adequate doses and to avoid salicylates.

Frequent doses of NSAIDs are given until the acute attack subsides. Treatment is then continued with lower doses, for 7-10 days.

Colchicine is highly effective but causes vomiting and diarrhea in many patients.

If there is a tendency for recurrence, lifelong treatment with preventive drugs such as Allopurinol, which reduces the production of uric acid, or Probenecid which increases the excretion of uric acid, is recommended.

Excessive purine intake and over-indulgence in alcohol should be avoided.

Gradual weight loss is encouraged in obese patients, and it is associated with a fall in serum urate.
