What is Parkinson's Disease?

Parkinson's disorder is a degenerative neurological condition in which the part of the brain that controls motor functions is affected. This region of the brain is called basal ganglia, and cells in this area secrete a chemical messenger (neurotransmitter) called dopamine. The lack of dopamine causes loss of nerve-cell transmission, and hence motor activity. This leads to the development of classical symptoms of Parkinson's like tremors, rigidity and slow movements (bradykinesia). It is also termed as 'paralysis agitans' or 'shaking palsy'. The condition is a progressive one, which means increased deterioration with time. Some researchers have linked it with the natural process of ageing, claiming that most people would suffer from it if only they lived long enough. As many as 50,000 patients are affected by this chronic condition every year, in the US alone. The incidence of Parkinson's disease is found to be slightly higher in men as compared to women.

Causes of Parkinson’s Disease

Pathophysiology

Symptoms of Parkinson's disease result from the loss of dopamine secreting nerve cells in some part of the midbrain. The nerve cells in one part of the midbrain called pars compacta secrete a vital chemical called dopamine. The brain's inability to produce enough of these cells leads to a depletion of dopamine, resulting in this disease.

The exact cause of this disorder still eludes researchers. However, a definite link between Parkinson's and genetic inheritance has been established.

Viral infections like influenza have also been associated with this disease.

The incidence is higher among people exposed to certain environmental toxins such as manganese, and drugs contaminated with a chemical, MPTP (1-methyl 4-phenyl 1, 2, 3, 6-tetrahydropyridine).

One suggested cause is exposure to an unrecognized environmental toxin. Such exposure may have occurred years before the onset of any sign or symptom.

Accumulating evidence suggests genetic susceptibility to the disease. Familial history is a major factor in the development of Parkinson's, and recent research has implicated chromosome 4 gene for carrying the disease to the following generations. Three genes for Parkinson's disease have recently been identified.

An increased incidence of Parkinson's disease has been noted in monozygotic or identical twins (twins produced from a single fertilized egg) of mothers who develop Parkinson's disease before the age of 50. Approximately 5% of parkinsonian patients have a familial form of the disorder.

Signs and Symptoms of Parkinson’s Disease

Tremors: Tremors begin unilaterally, and are the first feature to appear, in most cases. They are most severe when the person is at rest, and improve after initial movement. Although most common, this symptom is absent in about 30% of cases.

Rigidity: The muscles become stiff and movement is hampered, e.g. loss of arm-swing while walking.

Bradykinesia:Voluntary movements become slow.

Gait Changes: There may be a shuffling gait; where the patient takes short shuffling steps and also tends to trip over.

Fatigue: It has been found in about 50% of cases.

Postural Changes: There is a stooped, forward-flexed posture, which worsens with time.

Expressionless Face: The face often looks expressionless due to the involvement of facial muscles. This also leads to drooling.

Voice Changes: The patient has a soft, hoarse and impaired speech.

Difficulty in Swallowing: As the disease advances, swallowing becomes progressively more difficult and poses the risk of aspiration pneumonia.

Loss of Sexual Function: This occurs with the involvement of perineal muscles.

Besides these motor functions, other symptoms like depression, mood changes, memory loss, behavioral changes and sleep disturbances are also seen. Some studies have found depression to be present in about 20% to 80% of cases.

Diagnosis of Parkinson’s Disease

Unfortunately, there are no definitive laboratory tests available for the diagnosis of PD.

CT and MRI scans of the brain help in ruling out other similar central nervous system disorders, rather than confirming a PD diagnosis. The most reliable tool is the presentation of symptoms to the neurologist. The job of diagnosing this disease is best accomplished by a Parkinson's specialist.

Confirmatory evidence is the presence of microscopic structures in the brain called lewy bodies, which can be visualized at autopsy. Studies have revealed that the prevalence of lewy bodies is as high as 16% in people above the age of 80 and were never diagnosed with PD.

At present, the primary tool for diagnosis of and assessment of the severity of PD is the 'UNIFIED PARKINSON'S DISEASE RATING SCALE'.

Parkinson’s Disease Treatment

Conventional Therapy

Drugs used for tremors are benztropine, procyclidine and orphenodrine. These drugs are called anti cholinergic drugs. They have common side-effects such as dryness of the mouth, constipation, urine-retention and blurred vision. They may cause confusion and hallucinations in elderly patients.

Another drug Amantadine is used either alone or combined with an anti cholinergic drug. These drugs have side-effects such as restlessness, confusion, skin rashes, edema and disturbance of heart-rhythm. Many patients derive only transient relief.

Another drug Levodopa is given in combination with carbidopa for Parkinson's disease. The most common initial side-effects of this drug are nausea, vomiting, postural hypotension (decrease in blood-pressure with change of position), and occasionally, cardiac arrhythmia (disturbance of heart-rhythm).

Another class of drugs called Dopamine agonist (drug which stimulates physiologic activity of nerve cells normally stimulated by dopamine) which includes bromocriptine, pergolide, pramipexole and ropinirole are used for Parkinson's disease. But these drugs also have side-effects.

Surgical Treatment:

Destructive neurosurgical procedures were used to treat Parkinson's for some years, but their use declined with the advent of levodopa drug. These surgical procedures had complications like brain hemorrhage and visual-fluid defect. Such surgical procedures have been replaced by high-frequency stimulation of a selected location in the brain, using an implanted electrode and stimulator.

Other therapies being studied at present are botox injections, gene therapy, and neural transplantation where cells producing dopamine are transplanted in the midbrain.
