Infectious Mononucleosis, also known as Glandular Fever or Kissing Disease is an affection usually seen in teenagers and young adults. It is a viral infection caused due to the Epstein-Barr virus. This virus is usually transmitted through the saliva, and hence the name 'Kissing Disease'. However, it can also spread through other means like coughing, sneezing or sharing a drink.

In most cases, patients remain asymptomatic, even though they are affected. Once infected, the virus stays in the body for a lifetime. The disease is not as contagious as other viral infections. There is an incubation period of 4-6 weeks, after which the first symptoms are seen. It is generally a self-limiting illness, and only symptomatic treatment is required, in most cases. Infectious Mononucleosis generally occurs between the ages of 10 and 35.

Causes of Infectious Mononucleosis

The most common causes are the Epstein-Barr virus (EBV) and cytomegalovirus (CMV).

The disease is named because of the raised count of mononuclear leucocytes due to the infection. Atypical white blood cells are seen in peripheral blood slides.

The highest incidence is seen between the ages of 15 and 17.

Signs and Symptoms of Infectious Mononucleosis

Most cases of mononucleosis are asymptomatic throughout life. Symptoms usually appear after an incubation period of 6-8 weeks.

The most common symptoms are:

--Fever - This is always present, though it may vary in intensity.
--Sore throat - This is the most common symptom. It resembles streptococcal pharyngitis or any respiratory tract infection. Irregular white patches are seen on the tonsils and the back of the throat.
-- Swollen lymph nodes - Enlarged and painful lymph nodes are seen on both sides of the neck and in the armpits.
-- Fatigue - There is an unexplained feeling of tiredness.
-- Muscular aches and pains
--Loss of appetite

Less commonly seen symptoms are:

-- Enlarged spleen, which may give rise to abdominal pain
--Headache
-- Swollen tonsils

However, this test may not detect the virus in the first week of infection. An ELISA antibody test may be positive.
