What is Pre-Diabetes?

Pre-diabetes, also known as impaired glucose tolerance and impaired fasting glucose, is a condition characterized by high blood glucose levels but not high enough to be diagnosed as diabetes. People with pre-diabetes are at increased risk for developing Type II diabetes and various heart diseases.

In the US, about 40 to 45 million people in the age group of 40-74 suffer from pre-diabetes. 1 out of every 10 males and 25 females aged 12 to 19 have pre-diabetes.

Causes of Pre-diabetes

Insulin helps in digestion of glucose (sugar), which gives energy to the body. In pre-diabetes, the cells of the body show resistance to the action of insulin, and thus, glucose begins to build up in the blood stream. The exact cause of this resistance to insulin is uncertain, although excess weight and fatty tissue seem to be important factors. Most people with pre-diabetes are overweight or obese.

Symptoms of Pre-diabetes

Most people with pre-diabetes have no symptoms at all. However, some typical symptoms of diabetes might be seen in some patients. These symptoms include unusual thirst, frequent desire to urinate, blurred vision and extreme fatigue.

Diagnosis of Pre-diabetes

Who should be tested for pre-diabetes?

People with any of the following factors should be tested for glucose levels regularly:

• 45 years or older
• Excess weight with a BMI (body mass index) over 25
• A history of abnormal glucose tolerance test or impaired fasting glucose level
• A history of gestational diabetes or delivering a baby that weighed more than 9 lbs
• High triglycerides and low HDL cholesterol, central obesity and hypertension

Diagnosis of pre-diabetes is based on blood glucose levels. There are two different blood tests used to determine pre-diabetes.

• Fasting plasma glucose (FPG) test - The blood glucose level is measured after 8 hours of fasting. If the blood glucose level is between 100 - 125 mg/dL, it is called impaired fasting glucose, which suggests pre-diabetes.

• Oral glucose tolerance test (OGTT) - The blood glucose level is measured after a fast and then again 2 hours after drinking a solution containing 75 gms of glucose. Two hours after the drink, if the glucose level is between 140 - 199 mg/dL, it is called 'impaired glucose tolerance', which suggests pre-diabetes.

Treatment of Pre-diabetes

Conventional System

The conventional system uses the following drugs: biguanides like metformin (Glucophage); thiozolidinediones like pioglitazone (Actos) and rosiglitazone (Avandia); glimepiride (Amaryl); and insulin injections to treat this condition.

Type I diabetes, also known as insulin dependent diabetes mellitus (IDDM) is a type of diabetes that is characterized by lack of sufficient amount of insulin in the body. Without sufficient insulin, the blood glucose levels in the body rise. This is known as hyperglycemia.

Type I diabetes typically affects younger individuals and therefore, was previously known as juvenile diabetes. Most people with Type I diabetes show vague symptoms like weight loss, and thus, the disease goes undetected. People with Type I diabetes are at a higher risk of heart diseases, stroke, kidney failure, high blood pressure, blindness and nerve damage. The most life-threatening complication of Type I diabetes is diabetic ketoacidosis , which is characterized by nausea, vomiting and coma.

Causes of Diabetes Type 1

The cause for Type I diabetes is not yet known, but, the cells of the pancreas stop producing the normal amount of insulin needed to maintain a normal blood glucose level in the body.

In the US, 90-95% of the 21 million people with diabetes, have Type II diabetes. It mostly occurs after the age of 45. It is often preventable, as the main cause of this type of diabetes is obesity. If left uncontrolled, it can result in life-threatening complications.

Causes of Diabetes Type 2

Type II diabetes develops when the body shows resistance to insulin and thus, the glucose circulating in the blood is not digested. The exact cause of this resistance is not known, though many studies reveal that obesity has a major role to play in its development.

Family history, pre-diabetes, inactivity, excess weight and age are important risk factors noted in the development of Type II diabetes.

What is Gestational Diabetes?

Gestational diabetes, also known as glucose intolerance during pregnancy, is a type of diabetes that is revealed for the first time during pregnancy. Diabetes is a condition characterized by a high glucose (sugar) level in the blood. Normal levels of glucose are beneficial for the body to get energy. However, excessive glucose in the body can be harmful, especially during pregnancy. In the US, 3 to 8 out of every 100 pregnant women suffer from gestational diabetes. It usually resolves itself with eating healthy food and exercising regularly.

Gestational diabetes affects the mother in late pregnancy, usually in the fifth or sixth month of pregnancy (between the 24th & 28th week). It can affect the baby by causing macrosomia, commonly known as 'fat' baby. Babies with macrosomia suffer from conditions like jaundice, low blood glucose levels after birth, and a have a high risk of developing breathing problems, obesity and Type II diabetes.

Causes of Gestational Diabetes

The cause of gestational diabetes is still unknown. However, some studies reveal that insulin resistance occurs during pregnancy, which may cause gestational diabetes. Due to this resistance, the mother's body is not able to utilize the insulin produced. The glucose is not digested due to lack of insulin and thus, builds up in the blood to high levels known as hyperglycemia. Persistent hyperglycemia can lead to gestational diabetes.

Symptoms of Gestational Diabetes

Gestational diabetes usually does not cause any noticeable symptoms. It may on rare occasion cause typical symptoms of diabetes, like:

• Increased thirst (polydipsia)
• Increased urination (polyuria)
• Weight loss in spite of increased appetite (polyphagia)
• Fatigue
• Frequent infections of the bladder, vagina and skin
• Nausea and vomiting

Diagnosis of Gestational Diabetes

Gestational diabetes is diagnosed by the oral glucose tolerance test (OGTT).

In this test, the patient drinks a solution of 75 gm glucose, and the blood glucose levels are measured at various intervals. The levels are first measured before drinking the solution (fasting glucose) and then at an interval of 1, 2 and 3 hrs after drinking it. If blood glucose levels meet or exceed any two of the following levels, it is termed as gestational diabetes:

• Fasting glucose level - 95 mg/dL or higher
• 1-hour level - 180 mg/dL or higher
• 2-hour level - 155 mg/dL or higher
• 3-hour level - 140 mg/dL or higher

Treatment of Gestational Diabetes

Treatment of gestational diabetes includes special low calorie meal plans and exercise. It also includes daily blood glucose monitoring and insulin injections in patients with uncontrolled glucose levels. Gestational diabetes usually resolves after pregnancy. However, once there has been an episode of gestational diabetes, the chances of it returning in future pregnancies is very high (66%).

Conventional System

The only treatment in the conventional system of medicine is the intake of insulin. Insulin intake requires regular and prolonged monitoring. DIABETES INSIPIDUS

What is Diabetes Insipidus (DI)?

Diabetes Insipidus (DI) is a condition characterized by excretion of large amounts of severely diluted urine, and increased thirst. Here, the reduction in fluid intake does not have any effect on the output of urine. Diabetes insipidus should not be confused with diabetes mellitus, which results from insulin deficiency or resistance leading to high blood glucose. These conditions are unrelated, although they may have similar signs and symptoms. Diabetes insipidus is often referred to as 'water' diabetes to distinguish it from the more common diabetes mellitus or 'sugar' diabetes.

Types of Diabetes Insipidus

Diabetes Insipidus (DI) is divided into 4 types, each of which has a different cause and must be treated differently.

1) Neurogenic Diabetes Insipidus

Neurogenic diabetes insipidus is the most commonly occurring from of DI. It is commonly known as Central Diabetes Insipidus. It results from damage to the hypothalamus or pituitary gland, which disrupts the normal production, storage and release of ADH (Antidiuretic Hormone). ADH / Vasopressin is produced in the Hypothalamus and stored in the pituitary gland. It is released into the blood stream by the pituitary gland to regulate fluid balance in the body.

2) Nephrogenic Diabetes Insipidus

Nephrogenic diabetes insipidus occurs due to the inability of the kidneys to respond normally to ADH. Such malfunctioning of the kidneys can be caused by drugs like Lithium, polycystic kidney disease, sickle cell disease, renal failure, partial blockage of the ureters and inherited genetic disorders.

3) Dipsogenic Diabetes Insipidus

Dipsogenic DI is due to a defect in or damage to the thirst mechanism, which is located in the hypothalamus. This results in an abnormal increase in thirst and fluid intake, which suppresses ADH secretion and increases urine output.

4) Gestational Diabetes Insipidus

Gestational DI refers to diabetes insipidus that occurs during pregnancy. While all pregnant women produce vasopressinase in the placenta, which breaks down ADH, this assumes an extreme form in the case of GDI. Diabetes insipidus is also associated with some serious diseases of pregnancy such as pre-eclampsia, HELLP Syndrome and Acute fatty liver of pregnancy. Failure to treat these diseases promptly can lead to maternal or perinatal mortality.

Symptoms of Diabetes Insipidus

The symptoms of DI occur in the absence of high blood sugar and/or urine sugar and are sometimes associated with various other symptoms due to concomitant damage to other hormones or organs. Adults with untreated DI may remain healthy for decades as long as enough water is consumed to maintain adequate hydration. In children, DI can interfere with appetite, weight gain and even growth.

General symptoms of Diabetes Insipidus include:

• Excessive Urination throughout the day and night

• Extreme thirst which is often for cold water

• Nocturia

• Enuresis

• Dehydration and Potassium depletion

• Sodium imbalance

Diagnosis of Diabetes Insipidus

Tests are conducted to differentiate DI from other causes of excess urination.

Some of the tests that are commonly used to determine the type of DI and/or look for its cause are:

• Urinalyisis

• Measurements of ADH and other substances in blood and urine before and during a water deprivation or dehydration test.

• An MRI (Magnetic Resonance Image) of the brain to rule out pituitary and hypothalamus injury.

• Therapeutic trial of a synthetic anti-diuretic hormone known as DDAVP

• Genetic testing in cases with family history of DI

Treatment of Diabetes Insipidus

Conventional Treatment

The conventional line of treatment protocol largely depends on the cause of DI. Desmopressin is the choice of drug to treat most forms of DI, including Central DI and gestational DI.
