 Backache is a common musculo-skeletal disorder. The source of pain can be vertebrae, intervertebral joints (disc and facet joints), ligaments and fasciae. Causes of Backache
Back pain can be acute or chronic.
The following are the common causes:
Developmental defects, e.g. abnormal vertebral facets, socialization of L5 transverse process, spondylolysis or listhesis between L5-S1 vertebrae
Trauma to ligaments, muscles, vertebrae or annulus with disc prolapse
Infections: pyogenic, tuberculous, etc.

Symptoms of Backache
Persistent pain or stiffness along the spine
Localised sharp shooting pain in the neck, upper back or lower back, after strenuous activity

Investigations in Backache
Investigations include plain X-ray, CT, MRI and bone scan. CT and MRI are of special value in evaluating anatomical details; of the two, MRI is the modality of choice.
Radioactive bone scan is a useful screening procedure for the diagnosis of
inflammatory
(ankylosing spondylitis ) and
infective
disorders. Spondylosis, spondylolisthesis and prolapsed intervertebral disc are common findings and should not automatically be accepted as the cause of low back pain.
These apart, based on the diagnosis, appropriate haematological, microbiological, biochemical and immunological tests should be carried out.
A bone biopsy is indicated when the diagnosis of osseous pathology is not clear.
Treatment
Conventional Therapy
Allopathic medicine treats backache as a symptom by administering anti-
inflammatory
and analgesic medications. These include over-the-counter drugs like aspirin, ibuprofen (Advil) and naproxen (Aleve), and prescription drugs like tolmetin (Tolectin), diclofenac (Voltaren) and indomethacin (Indocin). Another type of NSAIDs are COX-2 inhibitors like celecoxib (Celebrex). These drugs aim to reduce inflammation and relieve pain.
Auxiliary Treatment of Backache
In acute back pain, bed rest is advised with hips and knees held in the flexed position.
Local heat and gentle massage are helpful.
Traction may be required in some cases.
Lumbosacral support (corset) may be worn when ambulatory.
Abdominal muscle strengthening exercises are helpful too.
In chronic cases, weight reduction, exercises to improve abdominal muscle tone and strength, and correction of posture and working habits are advised. Patients are told to sleep on a firm mattress.

Sudden jerky movements and over activity should be avoided.
A back massage once a week could help to some extent.
Relaxation techniques, yoga, and meditation may also be resorted to.
